Forms available in the library
Various Federal, Indiana, Kentucky, and Ohio tax forms are available in Steely Library. Forms can be picked up in the reference area on the plaza level. Forms and instructions are on the file cabinets to the left of the glass elevator. If you don't see the form you need, ask at the information desk to see the reproducible set of tax forms.
While we make these forms available to the public, we cannot help you complete these forms nor can we provide advice concerning tax preparation.
